Forms

Clients are encouraged to download, print and complete the Intake Forms prior to their first appointment.  These forms provide valuable information regarding confidentiality, client rights, fees, and suggestions for making your first appointment as comfortable and productive as possible.   Please bring the completed forms to your first session.  If there are any questions or concerns about the forms, you are welcome to address these with Dr. Ogletree before your session.

Fees

Individual Sessions (50 minutes) are between $100 and $150. Payment is expected at the end of each session. Ecclesiastical leaders will be billed monthly.

Payment

Clients may pay for their therapy sessions using the PayPal system. All major credit cards are accepted and the online transactions are secure. Personal checks and cash are also accepted on the day of one's appointment. To pay for an INDIVIDUAL therapy session.
